ALL COMPETITORS ARE REQUIRED TO SIGN THIS DOCUMENT
Parent or guardian must sign if participant is under the age of 18.
No entry will be accepted without this signed form.
All persons attending this event, whether as a spectator, competitor, studio employee or guest of the
organizers, shall be bound by the rules of "Manatee Madness Ballroom Dance Festival and Congress" (aka. Manatee Madness) and by participating in this event, automatically become obligated to adhere to them.
Manatee Madness, Archangel Michael Greek Orthodox Church, Holiday Inn Express Hotel, participating studios, attending vendors and other affiliates accept no responsibility for any loss or theft of articles left in changing rooms, ballroom, or hotel rooms, or for any loss or injury sustained by persons attending this event.
Manatee Madness will not be held liable for injuries sustained by persons attending this event, whether as a spectator, competitor, official, studio employee, or guest of the organizers. Everyone does so at his or her own risk.
The undersigned hereby expressly and irrevocably waives any claim or claims arising from any loss or injury
incurred at this event.
Manatee Madness rules provide that no Videotaping, of any Manatee Madness events will be allowed without prior written consent of the organizers. In consideration of all goods and services, receipt of which is hereby acknowledged, the undersigned hereby irrevocably gives and grants to Manatee Madness, the right to use, refer to and reproduce programs by means of video recordings and/or still photography in any and all media formats, hereinafter referred to as “reproductions”, edited and arranged as desired, using my name, voice, likeness, acts, poses, appearances and utterances as part of and in connection with my appearance in any Manatee Madness program: (a) in any advertising, publicity and promotion (b) the program and the products or services of any sponsor(s) thereof (c) to exhibit, transmit, distribute, sell and use in any aspect, reproductions of my performance or appearance at Manatee Madness and (d) otherwise use same in all fields and media throughout the world without limitations.
Manatee Madness will not be liable for any personal charges made at the hotel. All participants must pay their own personal charges, including, but not limited to phone charges, room service, etc.
There will be no smoking allowed in the ballroom or changing rooms.
